Transfer of healthy donor stool to restore gut microbiome diversity and function. FDA-approved for recurrent C. diff, experimental for other conditions. Can be done via colonoscopy, endoscopy, enema, or oral capsules.
Screen and select healthy donor (extensive testing for pathogens, metabolic health) or use FDA-approved commercial product
Recipient preparation: Bowel cleanse day before procedure (colonoscopy prep)
Stop antibiotics 24-48 hours before FMT
For colonoscopy FMT: Fresh/frozen stool processed and filtered, 250-500ml installed during colonoscopy
For enema FMT: 250ml stool slurry retained for 30-60 minutes
For oral capsules: Take 30-60 capsules (freeze-dried stool) over 1-2 days
Post-procedure: Probiotic foods, prebiotic fiber to support new microbiome
